abmeldung deregistration from germany

• i left germany in 2000 i have applied for a visa to visit family there and have been informed that i need a deregistration form can anyone help me to find out how to get one when i lived in germany i lived 23c esbach str 98708 gehren thuringen i need to know where in this town i can get one and how to contact them i do not speak german so find it difficult .
